Some plans let you use any dentist; other plans you must choose from their dental provider network. Plans that let you use any dentist cost more than plans with network you must use. The plans with networks, such as Employers Dental Service and TDA's lower cost option (both below), are a wise choice. These plans have no annual maximum benefit amount, no waiting period, and your out-of-pocket costs when you see a dentist is almost the same as if you had a higher cost plan.


Employers Dental Services

This is the plan to get if you currently do not have a dentist, or your dentist is in the EDS network, and you want to save significantly on dental procedures and routine maintenance. To apply for this plan, you must print the Employers Dental Services (EDS) Brochure & Application from the link below, complete, and submit by mail, fax, or email.

  • No deductible
  • No waiting periods
  • No annual maximum benefit
  • Must use network dentist
Rates:

  • $18 for a single adult; $11.90 for a single child
  • $29.60 for an adult + 1
  • $38.50 for an adult + 2
  • $47.75 for an adult + 3 or more

TDA Copay Plan endorsed by Blue Cross Blue Shield of Arizona

This is the plan to get if you currently do not have a dentist, or your dentist is in the TDA network, and you want to save significantly on dental procedures and routine maintenance. You can apply for the TDA-A800R Copay Plan using the link below.

  • No waiting periods
  • No annual maximum benefit
  • Must use network dentist
Rates:

  • $17 for an individual
  • $29 for an individual + 1
  • $45 for an individual + 2 or more

TDA dentalCOMPANION PPO/Indemnity Plan

This is the plan to get if you currently do not have a dentist, or your dentist is in the TDA - PPO network. This plan pays a scheduled amount for Covered Dental Procedures, which you can view by clicking the link and scrolling to page 11. You can apply for the TDA dentalCOMPANION PPO/Indemnity Plan using the link below.

  • $25 deductible for Basic and Major Services
  • 12 month wait for Major Services
  • $1,000 annual maximum benefit
  • Includes discounts for hearing, vision, and prescriptions

  • $56.47 for an individual + 1
  • $92.56 for an individual + 2

UnitedHealthOne Dental Plans

This is the plan to get if you have a dentist in the UnitedHealthOne network, or you have a dentist that is outside of the network. UnitedHealthOne's Dental Premier plan option pays a higher benefit for Out-of-Network providers. You can apply for the UnitedHealthOne Dental Premier or Dental Value Plan using the link below.

Both plans feature:

  • $50 deductible for Basic and Major services
  • 6 month wait for Basic services
  • 12 month wait for Major services
  • $1,000 annual maximum benefit

Main Differences:

  • Dental Premier pays a higher benefit for Out-of-Network Dentists
  • Dental Value pays a lower benefit for Out-of-Network Dentists

Delta Dental

Many dentists in Arizona are in the Delta Dental Provider Network. If you know your dentist is in this network, this is the plan to get, although this plan does allow you to see an Out-of-Network dentist.

This is a dental discount plan, like EDS and TDA above. Careington's plans is a national network, however. If you travel outside of Arizona often and need dental services while away, this is the plan to get.
