Blue Cross Blue Shield of Arizona offers the most popular individual and family health insurance plans available in the state.

The three PPO options that Blue Cross offers have deductibles that range from $250-$10,000. The deductible comes into the picture for major care such as hospitalization, surgery, and major testing. You are not required to meet your deductible in order to get doctor visits and prescriptions for just a copay.

Blue Optimum Plus PPO Plan

The most expensive Blue Cross plan, called BlueOptimum Plus, is a high-quality product offered at a competitive rate. You pay just $30 when you go to a primary doctor and $50 when you visit a specialist. Generic prescriptions cost a maximum of $15 for a 30 day supply, and brand name drugs range from $40-$120. Maximum out of pocket cost on this plan is the deductible amount plus $2,500.

Blue Essential Plus PPO Plan

The BlueEssential Plus PPO plan is the most affordable plan that Blue Cross offers. You pay just $30 to visit a primary doctor and $50 for a specialist, however you are limited to three office visits per year. (After three visits, you then pay the reduced rate that Blue Cross gets the visit for, which is usually $75-$100.) Generic prescriptions cost a maximum of $15, and brand name prescriptions cost $125. Your maximum out of pocket cost is the deductible amount plus another $4,000.

Blue Value Plus PPO Plan

The BlueValue Plus PPO plan is a very popular mid-range plan that Blue Cross offers. You pay $30 to visit a primary physician and $50 for a specialist. However you are limited to six office visits per year. (After six visits, you pay the reduced rate that Blue Cross gets the visit for.) Generic drugs cost a maximum of $15, and brand name prescriptions range from $40-$120. There is a separate $500 deductible for brand name prescription drugs. Your maximum out of pocket cost is the deductible amount plus another $3,000.

Blue Portfolio Plus HSA (Health Savings Account) Plan

Another popular Arizona Blue Cross plan is called Blue Portfolio Plus HSA PPO plan. We think this is a very smart plan option for various reasons. The BluePortfolio plan is a health savings account-compatible plan. With an HSA-compatible plan, the IRS allows you to open a health savings account (like an IRA) at a financial institution of your choice. You can put pre-tax money into the account, and the money grows with interest. You may take money out of the account to pay for qualified medical expenses. If you don't use the HSA money, it carries over from year to year.

The Blue Portfolio Plus HSA Plan is available in three deductibles: $1750, $3500 and $5500. On this plan, you do not get doctor visits or prescriptions for just a copay like on the PPO plans described above. Instead, you must pay the doctor and prescription cost yourself until you meet your chosen deductible. But you won't be paying the doctor or drug store their normal charge – you will be paying the discounted rate that Blue Cross pays. And the copay and prescription costs will apply toward your deductible. Once you've met your chosen deductible, Blue Cross will pay 100% of the rest of your medical bills for the policy year. This includes, but is not limited to, your doctor visits, prescriptions, lab tests, surgeries and hospitalizations.
