The file you download will have an extension at the end of its name of asp. Be SURE to change that extension to PDF. As an example, the file that appears on your computer may be called ifpApp.asp. — change it to ifpApp.pdf or you may not be able to open it.

If you have any difficulty with downloading, please call us at (800) 600-9663 and we'll email an application to you or put any applications you desire into the US Mail for you. If your state does not appear when you click the link, call us and we'll send the application to you.
